Job Description: As a Clinical Social Worker at Protalents, you will play a critical role in ensuring the health and well-being of our members. Your primary responsibility will be to provide intensive follow-up to those who require extra support to comply with their medical treatment.
Key Responsibilities:
- Telephonic Follow-ups: You will engage in regular phone calls with members to assess their situation and provide guidance on managing their condition.
- Home Visits: When necessary, you will conduct home visits to offer personalized support and connect members with additional resources.
- Care Coordination: You will collaborate with community agencies and other healthcare professionals to develop and implement comprehensive care plans.
Qualifications:
- Advanced degree in Clinical Social Work
- Expertise in behavioral health, chronic conditions, and disease management
- Proven skills in clinical assessment and crisis intervention
- Current professional licensure
- At least two years of experience in a Clinical, Behavioral, or Managed Care setting
