Dialysis Social Work Job Description

Centers Dialysis Care is looking for a skilled Renal Social Worker to join our team in Brooklyn, NY. As a Renal Social Worker, you will be responsible for providing clinical services to patients with End Stage Renal Disease and their families.

Key Responsibilities:
• Conduct psychosocial assessments of patients and their families
• Develop and implement individualized treatment plans
• Participate in patient care plan meetings
• Provide education and support to patients and their families
• Collaborate with healthcare teams to coordinate patient care

Requirements:
• Master's degree in Social Work from a Council on Social Work Education-accredited program
• Licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of New York
• One year of dialysis experience preferred
