The Physical Therapist (PT) is a key member of our rehabilitation team, responsible for evaluating, treating, and delivering skilled PT services to our clients. The PT is accountable for initiating services in a timely manner, screening, evaluating, and treating patients, developing treatment plans, setting goals, and educating families and caregivers. The PT is also responsible for documenting all services provided to ensure accurate and timely billing of therapy services.
Responsibilities- Develops treatment plans and provides treatment within practice guidelines and consistent with benefit and reimbursement levels to achieve optimal functional outcomes for patients.
- Completes documentation, including evaluations, daily notes, progress notes, discharge summaries, or monthly reports as required.
- Directs and supervises the work of assistants, rehab aides, and students as applicable, adhering to State and Federal guidelines.
- Participates in care planning meetings, rehab meetings, and family training to communicate patient progress.
- Educates staff, other healthcare professionals, and third-party payers on the benefits and functional outcomes of intervention.
- Follows organizational and facility policies and procedures to ensure employee and patient safety.
- Ensures equipment is in good working order prior to providing care to patients and follows procedures for reporting damaged equipment.
- Maintains a clean environment.
- Attends continuing education seminars and workshops to stay current with new clinical skills, programs, or techniques.
- Participates in provision of in-service training sessions or other educational presentations to enhance clinical and professional knowledge and skills.
- Maintains appropriate state licensure, certification, and credentialing.
- Submits billing, payroll, and expense documentation in a timely manner.
- Makes appropriate referrals to other members of the rehab team.
- Provides regular screening of residents and reviews restorative care plans on a quarterly basis.
- Provides brief updates on patient progress verbally or in writing to the Director of Rehab and/or during unit rounds and care plan meetings.
- Attends interdisciplinary meetings in accordance with the policies of the rehabilitation department of the pediatric unit.
- Acts as a liaison therapist to establish relationships and communication with outside school therapists treating our children and disseminate this information to specific care meetings.
- Serves as a clinical instructor when PT student affiliates at this facility.
- Minimum one (1) year of experience in a residential/assisted living or skilled nursing facility.
- Minimum one (1) year of experience with CVA, TBI, modalities, and splinting.
