Job Title: Physical Therapist

Description:

The Physical Therapist is responsible for evaluating, treating, and developing treatment plans for patients in a skilled nursing facility. This includes screening, evaluating, and treating patients, as well as documenting all services provided to ensure accurate and timely billing of therapy services. The Physical Therapist must also ensure that patient care follows the organization's guidelines, meets professional standards, and is in compliance with Federal and State guidelines.

Responsibilities:

  • Develops treatment plans and provides treatment within the bounds of practice guidelines and consistent with benefit and reimbursement levels to achieve optimum functional outcomes for patients.
  • Completes documentation, including evaluations, daily notes, progress notes, discharge summaries, or monthly reports, as required.
  • Directs and supervises the work of assistants, rehab aides, and students as applicable, with State and Federal guidelines.
  • Participates in care planning meetings, rehab meetings, and family training to communicate patient progress.
  • Educates staff, other healthcare professionals, and third-party payers on the benefits and functional outcomes of intervention.
  • Follows organizational and facility policies and procedures to ensure employee and patient safety.
  • Maintains a clean environment and ensures equipment is in good working order prior to providing care to patients.
  • Attends continuing education seminars and workshops to keep abreast of new clinical skills, programs, or techniques.
  • Participates in provision of in-service training sessions or other educational presentations to enhance clinical and professional knowledge and skills.
  • Maintains appropriate state licensure, certification, and credentialing.
  • Submits billing, payroll, and expense documentation in a timely manner.
  • Makes appropriate referrals to other members of the rehab team.
  • Provides regular screening of residents and reviews restorative care plans on a quarterly basis.

Requirements:

  • Minimum one (1) year of experience with the pediatric population.
  • Minimum one (1) year in a residential/assisted living or skilled nursing facility.
  • Minimum one (1) year of experience with CVA, TBI, modalities, and splinting.
