Job Summary:We are seeking a skilled Physical Therapist to join our team. The successful candidate will be responsible for providing direct patient care and physical treatment to adult patients.
Key Responsibilities:- Establish short and long-term rehabilitation goals according to an individualized patient treatment plan and measure progress in achieving these goals.
- Instruct patients and caregivers in activities to be carried out between treatments and after discharge.
- Coordinate and communicate the plan of care and findings with Rehabilitation Supervisors, physicians, and other disciplines as needed.
- Minimum of Bachelor of Science in physical therapy from an accredited institute of learning.
- Minimum of one-year experience in providing therapy treatments in a healthcare setting.
- Registered and licensed PT in NYS.
- Valid CPR certification.
