Job Summary

WestCare seeks a skilled and compassionate Case Manager to join our team. In this critical role, you will work closely with clients to develop personalized case management plans, ensuring they receive the necessary support to overcome substance use and mental health challenges.

Key Responsibilities

  1. Maintain a comprehensive understanding of the Behavioral Healthcare Home model and treatment principles.
  2. Participate as a member of the treatment team by implementing therapeutic community concepts.
  3. Serve as part of the case management team, working collaboratively to ensure clients receive holistic care.
  4. Collect and enter accurate HMIS data in a timely manner, providing vital insights for data-driven decision-making.
  5. Participate in agency Case Management activities and complete case management tasks as assigned.
  6. Develop a network of resources, including medical, housing, employment, and educational services, to meet clients' diverse needs.
  7. Conduct case management assessments, develop client case management plans, and incorporate these plans into clients' master treatment plans.
  8. Facilitate case management plans, providing transportation and accompaniment to appointments, securing medical records, and monitoring medication management.
  9. Oversee clients' medication management, ensuring prescriptions and medications are filled and renewed according to established protocols.
  10. Ensure funder requirements related to Communicable Disease testing and immunization are met and compliant with timelines.
  11. Cordinate services provided by ancillary agencies, participate in treatment planning and reviews, and consult with clients regarding case management plan goals and objectives.
  12. Evaluate clients' eligibility for public and other entitlements, assist with health insurance application and enrollment, and facilitate access to all eligible entitlements.
  13. Evaluate clients' documentation of personal identification, immigration status, and legal status, and assist clients in obtaining relevant documentation.
  14. Evaluate clients' level of life skills and functioning, providing individual and group counseling, training, and education in areas of need.
  15. Assist medical and other staff in securing healthcare and medications required by each client, providing oversight of the healthcare plan.
  16. Participate in discharge planning, helping clients secure stable, safe post-discharge housing, employment, and a stable source of income.
  17. Maintain accurate documentation of client case management activities.
  18. Collaborate with primary therapists to ensure implementation of behavioral management strategies.
  19. Attend and participate in training activities, staff meetings, and supervisory sessions.

Requirements

Certifications/Licenses

Education

Bachelor's Degree in a social service field, preferred. Ability to understand substance abuse and mental health treatment concepts, ability to understand and address needs of clients with Co-occurring Disorders, familiarity with the community's health and other resources.

Experience and Competencies

  1. Positive Mental Attitude.
  2. Basic computer literacy, including data entry.
  3. Valid driver's license.

Working Conditions

  1. Work is primarily performed in an office or residential treatment setting.
  2. Some outdoor activities may be required.

Physical Demands

  1. Requires mobility and physical activity: having an adequate range of body motion and mobility, to work in the office environment, including standing and walking (even and uneven surfaces), sitting for extended periods, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 30 pounds.
  2. Constant use of computer and telephone systems is required, which includes coordination of eye and hand, and fine manipulation by the hands (typing, writing, and working with files).
  3. Requires the ability to operate a motor vehicle.

  1. Requires the ability to collect and analyze complex numerical and written data and verbal information to reach logical conclusions.
  2. Requires the ability to work and cooperate with clients, co-workers, managers, the public, and employees at all levels in order to exchange ideas, information, instructions, and opinions.
  3. Requires the ability to work under time deadlines.
