About the Role
We are seeking a skilled Registered Nurse to join our team as a Home Care Transition Specialist. In this role, you will work closely with our clients and their healthcare teams to ensure a seamless transition from hospital to home care.
Key Responsibilities
- Assess patients' medical needs and suitability for home care services
- Develop and implement personalized care plans in collaboration with healthcare providers
- Coordinate the transition of care from hospital to home, including arranging for necessary medical equipment and supplies
- Provide education and guidance to patients and their families on managing chronic conditions, wound care, and medication administration
- Facilitate referrals to appropriate home care services and ensure compliance with healthcare regulations and best practices
Requirements
- Graduate of an accredited school of nursing and currently registered in New York State as a Registered Professional Nurse
- Home care experience preferred
- Reliable vehicle for travel to patients
