We are seeking a compassionate and detail-oriented Patient Care Coordinator to join our team at Urban Recovery. As a key member of our staff, you will play a vital role in supporting clients in their recovery journey.
Responsibilities:- Provide professional, courteous, and empathetic support to clients at all times.
- Communicate regularly with Counselors and Nursing staff regarding client issues and care.
- Encourage positive interactions and conversations amongst clients and staff.
- Transport clients throughout the facility and ensure they adhere to program activity schedules.
- Conduct room checks and census checks every 2 hours, including after meals.
- Observe clients for inconsistent behavior and report to appropriate staff.
- Assist in the de-escalation of behaviorally compromised clients and monitor client linen and laundry.
- Collect laundry bags of dirty clothing and prepare for housekeeping to wash.
- Deliver clean laundry back to clients and search and record new admissions belongings.
- Assist staff with screening clients and their property for contraband.
- High school diploma or equivalent.
- One (1) year of related work experience.
- Detail-oriented and great interpersonal skills.
- Strong communication skills and ability to be kind, empathetic, and emotionally intelligent.
- Ability to de-escalate hostile situations and willingness to learn.
