Job Overview: Protalents is seeking a highly skilled Clinical Social Worker to join our team. This role will be responsible for providing intensive follow-up to members exhibiting behavior or an environment that may predispose them to non-compliance with medical treatment.

Responsibilities:

  • Provide telephonic follow-ups and home visits as needed
  • Integrate and coordinate care with community agencies
  • Coordinate services depending on member necessity

Requirements:

  • Master's Degree in Clinical Social Work
  • Strong knowledge of behavior principles, chronic illnesses, and disease management
  • Knowledge of clinical assessment and crisis intervention
  • Current, unrestricted license to practice
  • Minimum 2 years of experience in a Clinical, Behavioral, or Managed Care field preferred
