Job Summary: We are seeking a skilled Occupational Therapist to join our team in providing high-quality home health services to patients in The Woodlands, TX area. As a Home Health Occupational Therapist, you will work closely with patients to restore their mobility and independence, promoting a high quality of life and movement potential.

Responsibilities:
Provide skilled occupational therapy services to patients in their homes
Conduct thorough assessments and develop personalized treatment plans
Implement and evaluate treatment plans to ensure optimal patient outcomes
Collaborate with healthcare teams to ensure seamless care coordination
Maintain accurate and timely documentation of patient progress

Requirements:
Active license in the state of TX required
OT Liability insurance in the state of TX required
Graduate degree or greater in Occupational Therapy
Home Health or Rehab experience preferred
Excellent communication and interpersonal skills
