Job Summary

We are seeking a highly skilled Clinical Coordinator to join our Transitional Housing team. The successful candidate will supervise a team of Case Managers and ensure the utilization of Harm Reduction and Housing First strategies to provide the highest quality care for clients.

Key Responsibilities
  • Supervise a team of Case Managers and provide regular, structured supervision meetings
  • Develop and implement individual service plans, ensuring comprehensive and timely documentation of client contact and progress
  • Assess and evaluate client functioning, providing clinical direction and leadership to the program
  • Coordinate delivery of care with multiple service providers, including outreach teams
  • Provide support and guidance to front-line staff in managing difficult situations and de-escalating conflict
Requirements
  • NYS Licensed Masters Social Worker (LMSW) required, with ability to obtain LMSW within six months of employment
  • Minimum three years of experience working with related populations, with previous supervisory and administrative experience preferred
  • Thorough clinical understanding of relevant service delivery concepts and structures, including Housing First and Harm Reduction models
What We Offer
