Clinical Coordinator, Outreach

About the Role:

We are seeking a highly skilled Clinical Coordinator to join our Outreach team at Breaking Ground. As a Clinical Coordinator, you will play a critical role in ensuring the highest quality care for our clients, utilizing Harm Reduction and Housing First strategies to promote successful housing placement.

Key Responsibilities:

  • Supervise a team of Case Managers to ensure effective case management practices
  • Provide regular supervision and support to direct reports
  • Develop and implement individual service plans, ensuring comprehensive and timely documentation
  • Assess and evaluate client functioning, providing clinical direction and leadership to the program
  • Collaborate with outreach teams to ensure successful housing placement and minimize return to homelessness
  • Provide support and guidance to front-line staff in managing difficult situations and de-escalating conflict

Requirements:

  • NYS Licensed Masters Social Worker (LMSW) required, with ability to obtain LMSW within six months of employment
  • Minimum three years of experience working with related populations, with previous supervisory and administrative experience preferred
  • Thorough clinical understanding of relevant service delivery concepts and structures, including Housing First, client-centered motivational interviewing, and Harm Reduction models
  • Ability to work successfully with diverse stakeholders and populations
  • Proficiency with Microsoft Office Suite programs, including Word, Excel, Outlook, and Microsoft Teams

What We Offer:

  • Competitive salary range: $64,000 - $68,000
