About the Role:


The Massachusetts General Hospital (MGH) is seeking a highly skilled and compassionate Substance Use Disorder Care Coordinator to join our Program for Substance Use and Addiction Services (PSAS). As a key member of our interdisciplinary team, you will play a vital role in supporting patients with substance use disorders and their families.


Key Responsibilities:



  • Assist patients in connecting with a range of addiction treatment services, including inpatient and outpatient programs, support groups, and counseling services.
  • Work closely with our clinical team to ensure seamless transitions of care and optimal patient outcomes.
  • Provide education and support to patients and their families on addiction treatment options, recovery resources, and coping strategies.
  • Develop and maintain strong relationships with community-based organizations and providers to ensure access to comprehensive care.
  • Document patient interactions, progress, and outcomes in our electronic medical record system.

Requirements:



  • High school diploma required; degree in a related field (e.g., social work, counseling, psychology) preferred.
  • At least 2-3 years of experience working in the mental health or addiction field, preferably with experience in care coordination or case management.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team in a fast-paced environment.
