We are seeking a highly skilled Physical Therapist to join our team at CenterWell Home Health. As a Physical Therapist, you will play a vital role in helping our patients recover from injuries, illnesses, and functional disabilities, and regain their independence.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients, with a focus on achieving the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' Plans of Treatment and the overall goals of the patient/rehab team.
- Adjust treatment as needed to achieve maximum results and confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
- Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans that reflect current plans of care and supervise/evaluate staff performance.
- Accurately and promptly document patients' care observations, interventions, and evaluations, and assure that interim orders received from physicians and physical therapists are promptly and accurately documented, submitted for physician signature, and implemented.
- Report patients' progress to physicians, Clinical Managers, staff, patients, and families, and submit evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation are required
