About this Role:


We are seeking a dedicated and compassionate Care Coordinator to join our team at First Health Care of Puerto Rico.



Key Responsibilities:


  • Provide case management and support to individuals in need of services, orientation, and/or crisis intervention.
  • Refer clients to the appropriate level of care after consulting with clinical staff.
  • Use clinical structured data to determine eligibility for program services based on member needs and benefits.
  • Document services authorization and coordinate services with providers.
  • Educate clients and their representatives about coverage benefits and community resources.


Requirements:


  • Effective communication and case management skills.
  • Ability to work in a fast-paced environment and prioritize tasks efficiently.
  • Knowledge of healthcare services and benefits.
  • Ability to maintain confidentiality and handle sensitive information.
