DescriptionPosition Details:Hours of typical operation are M-F, 7:00am - 4:00pm during training. Must be able to work a flexible schedule, as needed.Benefited position, coverage eligibility will begin 1st of the month following date of hire.Position Summary:The Clinical Pharmacist is responsible for the provision of safe, effective, and prompt medication therapy to District patients. Provide support of medication-use systems and deliver optimal medication therapy to patients with a broad range of disease states, provide direct patient care in coordination with physicians, nurses, and other hospital personnel. These clinicians are aligned with interdisciplinary programs and specialty services to deliver medication therapy and to ensure pharmaceutical care programs are appropriately integrated throughout the hospital. Participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to patients. Pharmaceutical care services include assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Serve as departmental resources to other departments, District personnel, or external groups as well as conduct patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Provide medication and practice-related education/training. Participation in the quality management program is expected with the goal of improving pharmacy services. Responsibilities may include working to establish and maintain long and short term goals for the Quality Management Program; monitoring and documenting Quality Improvement Projects for progress in meeting QI goals; and providing guidance and education to staff on Quality Management projects.RequirementsEducation:Graduate of an ACPE accredited College of Pharmacy.Licensure/Certification/Registration: Licensed (or eligible for licensure) in the state of OregonExperience:Clinical pharmacy experience in a hospital setting preferred.SummaryEmployment Requirements:To apply, please fill out an application, attach a cover letter, and resume.
