Job Summary: We are seeking a skilled Clinical Pharmacist to join our team at Lake Health District. As a key member of our healthcare team, you will be responsible for providing safe, effective, and prompt medication therapy to our patients.



Responsibilities:





  • Assess patient needs and provide individualized pharmaceutical care

  • Participate in interdisciplinary programs and specialty services to deliver medication therapy

  • Conduct quality improvement initiatives to improve medication-use processes and pharmacy practice

  • Provide medication and practice-related education/training to staff

  • Monitor and document quality improvement projects to ensure progress in meeting quality improvement goals



Requirements:





  • Graduate of an ACPE accredited College of Pharmacy

  • Licensed (or eligible for licensure) in the state of Oregon

  • Clinical pharmacy experience in a hospital setting preferred

  • Subject to satisfactory adjudication of background investigation and/or fingerprint check

  • Successful completion of 500-hour probationary period

  • Criminal background check and pre-employment drug screen required upon conditional job offer
