Description Upham's Home Health Care is seeking Full and Part-Time RNs to provide direct nursing care to Boston residents in their homes. Join our growing program Flexible schedules. Full orientation provided. Registered Nurse license and car required. Bilingual English/Spanish/Cape Verdean Creole or Vietnamese preferred. Duties: Completes initial evaluation visits, including history and physical exam.Regularly evaluates the patient's nursing needs.Develops nursing care plan and makes necessary revisions.Provides health services requiring substantial specialized nursing skills including appropriate preventative and rehabilitative nursing procedures.Utilizes other community agencies and resources to provide comprehensive patient care.Coordinates additional health services provided to patients.Discusses changes in the patients' condition and needs with the physician and other appropriate personnel.Supports the patients' family in meeting the physical, psychosocial and spiritual needs of the patient.Supervises Home Health Aides as required.Provides community consultation and health education when appropriate.Keeps accurate records of patients' progress.Has overall responsibility for maintaining his/her patient records according to the program's protocol.Participates in the regular case review meetings.Participates in the on-call system.Provides statistical data on patients as required by the program.Must provide a minimum of 25-30 patient encounters per week for a full-time position.Other duties as assigned. Qualifications QUALIFICATIONS: Must be a Registered Nurse licensed under the laws of the Commonwealth of Massachusetts.Ability to work with multicultural staff and participants.Background in geriatric or adult nursing preferredBi-lingual in English and Cape Verdean Creole is highly desirable.
