Job Title: Registered Nurse – Hospice (Case Management)
Location: California, West Virginia, Kentucky, and Pennsylvania
Schedule: Monday–Friday, 8:00 AM – 4:30 PM

Job Description:

We are seeking an experienced Registered Nurse with strong Hospice and HCHB experience to provide comprehensive, patient-centered care in a home health setting. The RN will manage a caseload, perform admissions, routine, PRN, and end-of-life visits, and collaborate with interdisciplinary teams to ensure high-quality care delivery.

Key Responsibilities:

* Conduct comprehensive assessments for high-risk patients
* Develop and implement individualized care plans in collaboration with providers
* Perform admission, routine, PRN, and death visits
* Coordinate care transitions and collaborate with physicians, social workers, and care teams
* Ensure appropriate, cost-effective levels of care
* Support patients with non-medical needs (housing, transportation, etc.)
* Maintain accurate clinical and documentation records
* Promote health, prevent complications, and intervene when needed
* Advocate for patient privacy, safety, and overall well-being

Requirements:

* Active RN License
* Associate's or Bachelor's Degree in Nursing
* Hospice and HCHB experience required
* Strong clinical judgment and decision-making skills
* Case Management certification preferred

Pay: $* per hour
