Boro Park Center for Rehabilitation and Healthcare is seeking a highly organized and motivated MDS Assessor to join our team in Brooklyn, NY.

Key Responsibilities:

  • Maintaining and updating all care plans and assessments as required
  • Monitoring and auditing clinical records, ensuring accuracy and timeliness
  • Informing the Director of Nursing of persistent issues related to non-compliant documentation
  • Protecting the confidentiality of Resident and Facility information at all times

Requirements:

  • Valid New York RN License
  • Excellent communication and computer skills
  • Ability to work in a team environment
