Job Summary:
We are seeking a highly skilled and compassionate Care Management Team Leader to join our team at National Youth Advocate Program. As a key member of our care management team, you will provide mentoring, case consultation, and support to program staff, working closely with youth, families, and community partners to ensure the best possible outcomes for our clients.
Responsibilities:
- Provide mentoring, case consultation, and support to program staff on a daily basis.
- Travel as needed to provide and support the provision of services to youth and families.
- Coordinate, facilitate, and monitor services for youth and families in the community, including mental health, medical, educational, psychological, vocational, independent living, crisis, and social services.
- Establish and maintain strong relationships with assigned foster parents and families of origin.
- Support and assist program staff in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement.
- Guide and support program staff in safety assessment, crisis response, and behavioral stabilization services as required to ensure safety and stability for youth served.
Requirements:
- Master's degree in social work or a comparable human services field from an accredited institution.
- 3 years of experience working in direct service with youth and families.
- Valid driver's license, reliable transportation, automotive insurance, and a good driving record.
- Proficient use of desktop and laptop computers, smartphones, and tablets, as well as software including word processing, spreadsheet, and database programs.
