Job Title: Case Manager, Wellness

At The People Concern, we are dedicated to empowering the most vulnerable among us to rebuild their lives. As a Case Manager, Wellness, you will play a critical role in supporting our clients' housing stability, self-sufficiency, and community integration.

Job Summary:

This is a full-time, non-management position that requires a strong understanding of issues faced by homeless individuals, including mental illness, substance abuse, and domestic violence. The successful candidate will have experience in group facilitation, social services, and community-based settings.

Key Responsibilities:
  • Develop effective, trusting relationships with clients using a client-centered approach.
  • Link participants to domestic violence and homeless family services resources.
  • Provide linkage to shelter, mental health, and health services, detox/rehabilitation programs, and social service advocacy.
  • Facilitate weekly resource tables outside of the office.
  • Connect participants to employment, benefits, and documentation.
  • Assist the Case Management team with orientation and training.
  • Create and facilitate wellness groups for participants using SAMHSA's guide to wellness.
  • Recruit facilitators to provide wellness groups and resource presentations.
  • Conduct monthly surveys, review data, and make changes to the program as needed.
  • Maintain accurate client records and statistical data.
  • Oversee Narcan distribution and reporting.
  • Participate in case file reviews, staff meetings, supervision, team meetings, training, and retreats.
  • Participate in ongoing self-evaluation, program evaluation, and development/modification.
  • Provide crisis intervention using Non-violent Crisis Intervention techniques.
  • Communicate effectively and promptly with management, peers, collaborators, and clients.
Requirements:
  • Bachelor's degree preferred; or a minimum two years' experience working with homeless individuals, preferably those living with mental illness and/or substance addictions.
  • One year experience in group facilitation.
  • Demonstrated knowledge of issues faced by the population served.
  • Knowledge of social services agencies, community, and housing resources.
  • Able to work in a high-tolerance Harm Reduction model.
  • Ability to self-motivate, learn quickly, and work effectively with a wide range of constituencies.
  • Experience working in a community-based setting and as part of a team.
  • Skilled in non-violent crisis intervention.
  • Detail-oriented with strong time management, organizational, written, verbal, and computer skills.
  • Current, valid California Driver's License with an acceptable driving record and reliable vehicle.
  • Comfortable, able, and willing to drive/transport program participants in agency vans.
  • Able to provide cleared TB test and pass a criminal background check.
Preferred Qualifications:
  • Bilingual preferred.
  • Bachelor's degree preferred.
Work Environment:

This is a combination of field and office environment. The successful candidate will be required to sit, stand, bend, and occasionally lift or carry up to 35 pounds. On occasion, they will need to walk or drive to different local sites. The work environment will necessitate working in busy and loud environments, exposing the candidate to elements like cold, heat, dust, noise, and odor. The candidate may need to bend, stoop, twist, and sit throughout the day.
