Job DescriptionWe are seeking a skilled Nurse Practitioner to join our team-based primary care practice in Mesa, AZ. As a key member of our healthcare team, you will provide high-quality, patient-centered care to individuals of all ages. Our practice offers a robust clinical and operations framework, supporting your ability to take care of patients and providing opportunities for knowledge-sharing and professional growth.
Key Responsibilities:- Provide comprehensive health history and physical assessments to patients and families
- Develop and implement individualized care plans to address acute, chronic, and potential health problems
- Collaborate with healthcare teams to ensure seamless care coordination and population health management
- Stay up-to-date with the latest clinical guidelines and best practices in primary care
- Graduate and certified from an accredited Nurse Practitioner program
- Two or more years of outpatient clinical experience as a Nurse Practitioner (preferred but not required)
- Current, unrestricted license in practicing state or be eligible for licensure
- Active Drug Enforcement Agency (DEA) registration or be eligible for registration
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
