Job Title: Licensed PT - Patient Care Expert

Clinical Medical Service seeks a highly skilled and licensed physical therapist to serve as a patient care expert. As a key member of our healthcare team, you will work collaboratively with physicians, nurses, and other professionals to deliver exceptional patient care in the comfort of their own homes.

1. Develop and implement individualized plans of care that meet the unique needs of each patient.
2. Conduct regular home visits to assess patient needs and provide treatment as prescribed by healthcare professionals.
3. Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
4. Maintain accurate records of patient progress and treatment outcomes.
5. Participate in continuing education and professional development to stay current with best practices in physical therapy.

• Graduate from a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited physical therapist curriculum.
• Current licensure in the state of Puerto Rico.
• At least 6 months of experience in physical therapy.
• Ability to exercise initiative and independent judgment.
• Strong communication and interpersonal skills.
• Valid Professional Register, Vaccines Record for HepA, Varicela, Flu, and Covid 19, BSLS/CPR, Law 300, Particle Test, Health Certificate, Good Conduct Certificate, Valid Driver's license.
