- Provide crisis intervention and protective services as needed, including guardianship and resource utilization.
- Collaborate with multidisciplinary teams to ensure timely discharge and coordinate post-hospital care.
- Act as an advocate for patients and families, connecting them with community resources and services.
- Participate in clinical performance improvement activities and contribute to the collection and reporting of financial indicators.
- Masters Degree in Social Work from a CSWE-accredited program.
- Licensure in Illinois as a Licensed Social Worker (LSW).
- Excellent interpersonal and organizational skills, with the ability to prioritize and manage a caseload.
- Self-direction and analytical skills to collect, analyze, and interpret data, resolve problems, and negotiate in sensitive situations.
