We are seeking a skilled Case Manager to join our team at Crosswinds Counseling & Wellness. As a Case Manager, you will provide quality case management services to clients in Osage county, utilizing a strengths-based approach to support youth, adults, and their families.
Key Responsibilities- Provide direct case management services to child and adult clients and their families, focusing on maintaining stability and residence in a family environment.
- Meet or exceed the billable hour standard of client contact hours set for the position.
- Assist clients and their families in identifying, securing, and maintaining community resources.
- Coordinate referrals and delivery of services among schools, community agencies, and within the organization to ensure clients receive necessary services.
- Develop and implement comprehensive, individualized plans of care based on strengths assessments.
- Maintain accurate and complete records of client services, submitting daily appointment sheets, progress notes, treatment plans, client status reports, timesheets, and other work-related paperwork in a timely manner.
- Associate's degree in psychology, social work, counseling, or a related field.
- Two years' experience in the health and human services field.
- Ability to work with persons with serious emotional disturbance.
- Demonstrated interpersonal skills and ability to react effectively in human service situations.
- Knowledge and skills regarding social, emotional, physical, and educational needs of students and families.
- Ability to provide data-driven decision-making.
- Computer experience in Microsoft Office programs and basic computer fluency.
This is a full-time position with regular travel within Region III to provide case management services. The ideal candidate will possess a strong understanding of community resources and the ability to access these services for the benefit of clients.
