Occupational Therapist

At Nueva Vida De La Salud Dba Cms Home Care Noreste, we are committed to providing exceptional home health care services to our patients. As an Occupational Therapist (OT), you will play a vital role in administering occupational therapy to patients in their place of residence. Your primary responsibility will be to provide intermittent occupational therapy services under the direction and supervision of the Director of Nursing.

Key Responsibilities:

• Administer occupational therapy services to patients in their place of residence
• Develop and implement personalized plans of care in collaboration with the interdisciplinary team
• Conduct assessments and evaluations to identify patient needs and goals
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

Requirements:

1. Graduate of an Occupational Therapy curriculum accredited by the American Occupational Therapy Association (AOTA)
2. Current licensure in the state(s) in which practicing
3. Two years' experience in occupational therapy, preferred
