Position Summary
As an Independent Optometrist at Sam's Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being. You will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule, keeping 100% of exam/professional fees. You can also opt into equipment/instrumentation and administrative/Opt-Tech support services (if state allows) for a fair market value.

Minimum Qualifications
• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9

About the Role
You will receive PTO and/or PPTO that can be used for vacation, sick leave, holidays, or other purposes. The amount you receive depends on your job classification and length of employment. It will meet or exceed the requirements of paid sick leave laws, where applicable.
