We are seeking a skilled Physical Therapist to join our multidisciplinary team in providing high-quality community-based care to patients in the Laurel, MD area.
The ideal candidate will have a strong background in geriatric rehabilitation and experience working in outpatient settings. A Doctor of Physical Therapy degree (or equivalent standing) and eligibility for Maryland state licensure as a Physical Therapist are required.
Key Responsibilities:
- Perform comprehensive assessments and develop individualized treatment plans for patients with various physical therapy needs.
- Deliver evidence-based care in a home-based setting, prioritizing patient-centered care and collaboration with other healthcare professionals.
- Document patient progress and maintain accurate records.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
- Doctor of Physical Therapy degree (or equivalent standing).
- Eligibility for Maryland state licensure as a Physical Therapist.
- Minimum 20 hours per week of direct patient care required.
- Regular and consistent weekly availability (3 weekdays) necessary.
