About the Role:We are seeking a skilled Care Manager to develop, assess, and facilitate complex care management activities for our members. As a Care Manager, you will provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.
Key Responsibilities:- Evaluate member needs, barriers to accessing care, and social determinants of health to identify priority areas for intervention.
- Develop and implement ongoing care plans and service plans in collaboration with providers to address member needs.
- Identify problems and barriers to care and provide appropriate care management interventions.
- Coordinate between members, caregivers, and care providers to ensure person-centered care and services.
- Provide ongoing follow-up and monitoring of member status, progress towards care plan goals, and revise or update care plans as necessary.
- Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
- Collect, document, and maintain member information and care management activities to ensure compliance with regulatory requirements.
- Provide education to members and their families on disease processes, care options, referrals, and healthcare benefits.
- Contribute to quality improvement initiatives and provide feedback to leadership on opportunities to enhance care delivery.
We are looking for a qualified Care Manager with a compact RN license, experience working with patients in behavioral health or obstetrical settings, and preferably located in the Eastern Time Zone. A Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience are required.
