Medication Refill Coordinator

At Alivia Health / Farmacias Plaza, we are seeking a dedicated Medication Refill Coordinator to join our team. In this role, you will be responsible for supporting patients with on-time medication refills for conditions included in the Centers for Medicare and Medicaid (CMS) STARs Program.

The primary goal of this position is to ensure that patients receive their necessary medications in a timely manner. To achieve this, you will work closely with Pharmacy Benefits Managers (PBMs) and Managed Care Organizations (MCOs) to understand their unique tools and programs for pharmacies.

Your key responsibilities will include:

• Coordinating medication refills with patients and healthcare providers
• Identifying potential barriers to adherence and developing strategies to overcome them
• Collaborating with PBMs and MCOs to optimize medication synchronization and 90-day conversions
