Job Description

Housing Works seeks a skilled Healthcare Navigator to support our Health Services division in providing quality care to individuals living with HIV/AIDS.

Key Responsibilities

  • Assist program director and lead navigator with administrative tasks
  • Collect and analyze data for the program, including SAMHSA Youth Questionaries and EBP's Assessments
  • Conduct mapping, outreach, recruitment, and linkage to care activities
  • Provide pre and post HIV test counseling and risk assessments
  • Assist with accompaniment, transportation, and follow-up services

Requirements

  • BA in human behavior or related field
  • Experience working in social services, particularly with homeless populations
  • Strong data collection and analysis skills
  • Excellent communication and interpersonal skills
