Job Title: Case Planner

At MercyFirst, we are committed to providing exceptional care and support to children and families in need. As a Case Planner, you will play a vital role in ensuring the well-being and safety of our clients.

Job Summary:

The Case Planner will work with children and youth who reside in foster homes, providing intensive support to ensure their progress towards permanency. This includes conducting regular casework sessions, home visits, and maintaining contact with birth parents and foster parents.

Responsibilities:
  • Provide intensive support to children, therapeutic foster parents, and birth families
  • Conduct regular casework sessions and home visits
  • Maintain regular contact with birth parents and foster parents
  • Document all contacts and case activities in the State system of Connections and Agency patient tracking systems
  • Prepare all required documentation on each case, including progress notes and psychosocial reports
  • Attend legal proceedings, representing the agency and client's best interests
  • Actively participate in staff training and supervision meetings
  • Arrange, monitor, and document family and/or sibling visits
  • Respond to emergencies as needed and/or directed
Requirements:
  • Bachelor's degree in social work or related field
  • 1 year of related work experience in serving children and/or families
  • Knowledge of and/or training in child development stages and family dynamics
  • Excellent communication skills (verbal and written)
  • Proficiency in Microsoft Word and Outlook
  • Ability to engage with clients
  • A valid driver's license is strongly preferred
