Clinical Social Worker

At Protalents, we prioritize diversity and inclusion. As a Clinical Social Worker, you will play a crucial role in providing intensive follow-up to members who may be at risk of non-compliance with medical treatment, leading to increased hospitalizations and worsening mental health outcomes.

The ideal candidate will possess a Master's Degree in Clinical Social Work and hold a current, unrestricted license to practice. A minimum of 2 years of experience in a clinical, behavioral, or managed care setting is preferred, but not required.

As a Clinical Social Worker, your responsibilities will include:

• Providing telephonic and in-person follow-ups to members in need
• Collaborating with community agencies to coordinate care and services
• Developing and implementing individualized plans to promote member engagement and adherence to treatment
