As a key member of our leadership team, you will be responsible for overseeing the daily operations of our care management program, ensuring high-quality client service delivery, and complying with program grants and contracts.
Key Responsibilities:- Ensure clinical oversight and high-quality client service delivery
- Enhance and maintain a sustainable Enhanced Care Management (ECM) program
- Prepare and oversee all necessary ECM components, including client files, documents, protocols, and procedures
- Train and modify assessment protocols in accordance with best practices and health plan regulations
- Review client case files for quality, timeliness, and accuracy
- Ensure all documentation is complete and signed
- Monitor and ensure staff service recordings, including accurate billing, are entered promptly into designated software systems
- Oversee and monitor program compliance to Wise & Healthy Aging policies and procedures
- Provide one-on-one clinical supervision of Care Coordinators, interns, and volunteers
- Facilitate biweekly staff meetings to review cases and coordinate training
- Master's degree in social work, psychology, public health, or a related field
- Licensure is required; a licensed clinical social worker and licensed psychologist are preferred
- Experience supervising employees who conduct fieldwork
- Strong clinical, interpersonal, verbal, and written communication skills
- Ability to multitask effectively and prioritize work efficiently with competing deadlines
- Valid driver's license, auto insurance, and satisfactory criminal background check are required
