Addiction Therapist

• Provide case management services to support positive treatment outcomes
• Lead therapy groups and provide individual therapy as needed for SUD and/or mental health disorders
• Develop patient-centered treatment plans based on screening tools and bio psychosocial assessments
• Collaborate with medical staff and treatment teams
• Consult with family members and referral sources to coordinate patient care

• Strong clinical, verbal, and writing skills
• Engaging and accountable approach with patients
• Flexibility and creativity as a team player
• Minimum 2 years experience in the SUD field, with group treatment experience preferred
• Electronic medical record experience and knowledge of recovery modalities
