**Prime Providers** is seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team. As a key member of our home-based care team, you will provide high-quality, personalized care to patients in the comfort of their own homes.

**Responsibilities:**

• Provide direct patient care under the supervision of a registered nurse

• Implement treatment plans initiated by the registered nurse

• Maintain accurate and timely documentation

• Educate patients and their families on diagnosis and treatment plans

• Prepare and coordinate clinical notes and progress reports

• Qualifications:

• Current CA Vocational Nurse License

• Acceptance of new graduates under the Flex Waiver Program

• Current PPD or chest x-ray if applicable

• Current State license, CPR certification, and valid driver's license for local travel
