Job Summary

The Nurse Practitioner is a key member of our interdisciplinary care team, responsible for providing high-quality, patient-centered care to our nursing home residents. This role requires a strong commitment to improving the health and quality of life of our members.

Key Responsibilities:

  • Provide proactive primary and acute care to members, including assessing needs, creating individualized care plans, and coordinating care to ensure smooth transitions across settings.
  • Manage members with acute changes in condition in collaboration with primary care physicians and the interdisciplinary care team.
  • Review and manage pharmacotherapy for each member, collaborating with clinical pharmacists and physicians as needed.
  • Prescribe appropriate medications, including controlled substances in accordance with NYS law.
  • Identify, treat, and document changes in condition, including ordering, reviewing, and following up on treatments and diagnostic testing.
  • Review nursing home reports, logs, medical records, and other relevant documents daily.
  • Complete timely health risk assessments and routine monthly comprehensive assessments.
  • Ensure appropriate and timely utilization of services requests, such as consultation with specialists.
  • Coordinate care across various healthcare settings and attend nursing home meetings and interdisciplinary care team meetings as needed.
  • Communicate with nursing home staff, the interdisciplinary care team, and family or designated representatives regularly to discuss changes in condition and/or the care plan.
  • Discuss and educate members and designated representatives on advance care planning.
  • Act as a point of contact for members, family or designated representatives, throughout the transition process, change in condition, and routine updates.
  • Work with the interdisciplinary care team to optimize the health and functional status of members, enhance pain management strategies, and improve the quality of life of members.
  • Provide clinical education and support to nursing staff as needed and ongoing education and support to members and family or designated representatives regarding their health status and established goals.
  • Participate in pre-planned marketing events with nursing home residents and families and participate in an 'on call' program providing telephonic and/or on-site assessments and interventions for members after hours and on weekends.
  • Manage the skilled day process.
