About the RoleThe Renal Social Worker will play a critical role in providing psychosocial assessment, treatment, and referral services to patients and their families. This includes conducting quarterly assessments, documenting patient status, and participating in patient care plan meetings. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.
Responsibilities- Conduct psychosocial assessments and develop treatment plans for patients and their families
- Provide referral services to community agencies for follow-up psychosocial care
- Participate in patient care plan meetings and contribute to the development of individualized care plans
- Document patient status on a quarterly basis and as needed
- Interpret facility policies and procedures to patients and families
To be considered for this role, you must have a Master's degree in Social Work from a Council on Social Work Education-accredited program and be licensed by the State of New York. One year of dialysis experience is preferred, but we are willing to train the right candidate. A strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care are essential for success in this role.
What We OfferCenters Dialysis Care offers a competitive salary range of $70,000 - $85,000, depending on experience. We also offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 401(k) plan. Our facilities are located in Brooklyn, NY, with multiple locations throughout the Tri-State Area.
