Dietitian Specialist

At Quality Dialysis Center, we are seeking a highly skilled Nutrition Care Coordinator to join our team. As a key member of our dialysis team, you will be responsible for providing comprehensive nutritional care to our patients.

Key Responsibilities:

• Initiate nutritional care plans for patients within one month of admission
• Collaborate with physicians to develop diet prescriptions
• Conduct nutritional assessments and develop care plans
• Provide nutritional counseling to hemodialysis patients
• Participate in the development and implementation of facility policies for nutritional care
• Maintain dietetic registration and continuing education hours
• Review monthly lab work and counsel patients accordingly
• Maintain current progress notes and ensure compliance with ESRD regulations
• Share nutritional information with multi-disciplinary team members
• Communicate with hospitals, long-term care facilities, and other healthcare providers as necessary

Requirements:

• Registered Dietitian (RD) certification
• Experience in dialysis nutrition care
• Strong communication and teamwork skills
